Pathways Community Center

The PATHWAYS concept is unique: to create in one location, a single point of entry that serves as a "pathway" to a continuum of care that will provide stability, supportive services, and shelter, to renew broken lives. As York County’s population continues to grow, so too does the number of people in crisis. Meeting this increasing demand for assistance has been an overwhelming task for our community’s service agencies. To help meet this challenge, a group of business, church, government, and community leaders has been working with existing agencies on a new model that offers access to all the critical services for our struggling adults and families at a SINGLE location in Rock Hill.

Rebound Behavioral Health

Rebound Behavioral Health is an adult inpatient treatment center and substance abuse rehab program nestled in a peaceful, wooded estate. Rebound Behavioral Health features a 63-bed inpatient treatment program for men and women aged 18 and older. The alcohol and drug detox program offers high-quality medically-monitored detox.

PARTIAL HOSPITALIZATION PROGRAM - The PHP is an ideal level of care for adults who can benefit from intensive step-down support following the completion of inpatient treatment or whose needs are not severe enough to warrant enrollment in our inpatient program.

New Hope Carolinas

New Hope Carolinas is a Psychiatric Residential Treatment Facility (PRTF), fully accredited by the Joint Commission.
This setting also operates a fully accredited private school and offers comprehensive medical/dental care and recreational therapy services on site. New Hope Treatment Centers, nationally recognized for behavioral healthcare, provides comprehensive residential treatment to children and their families.

LEARNING ENRICHED AUTISM PROGRAM (LEAP): Designed to provide male youth with Autism Spectrum Disorder in crisis with immediate, short-term stabilization and assessment services, LEAP provides a highly specialized environment of care, including individual rooms, muted colors throughout the unit, and alternative / augmented communication systems. In addition, each resident receives individual and group, speech and language, physical and occupational therapy services as indicated, and education services.

For nearly fifty years, Keystone has offered hope and affordable professional care to persons in our community who find themselves in difficult times due to health problems from Substance Use Disorders.  Keystone Substance Abuse Services is the county’s largest provider of treatment and prevention services. Keystone is internationally accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF) and adheres to the definition of addictive disease as developed by the American Society of Addiction Medicine (ASAM) and the standards for service placement. Keystone is a charter member of Behavioral Health Services Association of South Carolina. The South Carolina Department of Health and Environmental Control and the South Carolina Department of Alcohol and Other Drug Abuse Services annually inspect Keystone.

To provide services 24/7/365 within 60 minutes of contact with the CCRI team to meet the mental health needs of residents of SC

To enhance the crisis services array to include a statewide community crisis on-site emergency psychiatric screening and assessment.

Providing adults and children with clinical screening either in person at the location of the crisis, in persona at a CMHC clinic, via phone, or via telehealth communication software. Program Goals: To provide access and link patients to appropriate levels of care and divert from unnecessary hospitalization and incarcerations.

HEALTHY CONNECTIONS - South Carolina Medicaid

Healthy Connections is South Carolina's Medicaid program. It is a medical assistance program that helps pay for some or all medical bills for many people who cannot afford medical care. The program also assists individuals who are over 65 or have a disability with the costs of nursing facility care and other medical expenses. Eligibility is usually based on applicants' income and assets. Healthy Connections is administered by the South Carolina Department of Health and Human Services (SCDHHS) and pays medical bills with state and federal tax money.

HOW DO I APPLY?  To apply for Healthy Connections, you will need to complete and submit an application. You may apply online at apply.scdhhs.gov. SC Thrive is available to assist you with the online Healthy Connections application and can provide an assessment of your household's most likely health coverage options. Please contact their helpline at (800) 726-8774 or visit them online at scthrive.org if you have questions or need assistance.

You may also apply in person at your local county office or contact our main customer service line at (888) 549-0820. In addition, you may apply in person at federally qualified rural health centers and at most hospitals. TTY: 888-842-3620

FAMILY CONNECTION SC

Family Connection of South Carolina has a long history of providing parent-to-parent support to families of children with disabilities or special healthcare needs. Each year, hundreds of families in our state are connected with veteran "mentor" parents who have walked a similar journey.

Our healthcare team can help you with questions related to private insurance, Medicaid, TEFRA, appeals, waivers, and managed care information. Guardianship

Guarding, Advocating & Protecting Adults with Intellectual and Developmental Disabilities.
  • Guardianship involves the appointment of an individual (a guardian) by the Probate Court to handle the personal, custodial, and healthcare decisions for an incapacitated adult (your child).
  • A Conservatorship involves the Probate Court appointing an individual (a conservator—you in most cases) to manage an incapacitated adult’s (your child’s) financial affairs or property.
  • Power of Attorney is a legal document, also filed in Probate Court, which establishes certain legal powers to act on the behalf of an individual (your child). These documents are for a specific function like medical or financial affairs, and end at a specific time, or after a specific function is carried out.

MHASC and its affiliates are the single largest non-profit sponsoring housing for the psychiatrically disabled in South Carolina. 8. Most units thought the Department of Mental Health Program (through Catawba Mental Health for this area) are going to be given priority to families or family-based.

There is an optional state supplement for rent for those who have benefits due to mental illness. Inquire at the Social Security office or with Catawba Mental Health.
